About KC Placement Finder
KC Placement Finder helps hospital case managers and families find places for patients leaving the hospital in the Kansas City area. It covers nursing homes, long-term acute care hospitals, inpatient rehab, medical respite, and recovery housing in Jackson, Clay, Platte, and Cass counties in Missouri, and Johnson and Wyandotte counties in Kansas.
Who runs it
KC Placement Finder is a project by Ronit Patel, a medical student in the Kansas City area. It is an independent project. It is not affiliated with Medicare, the Centers for Medicare & Medicaid Services (CMS), any hospital, nursing home, or other facility listed here.
Where the information comes from
- Medicare (CMS): facility lists, certification, ratings, fines, and quality measures, from the federal Provider Data files published in September 2026.
- Facility websites and phone numbers: checked against each facility's own site where one is available.
- Public directories and news: used to find programs such as recovery housing and medical respite. Each listing shows its source.
- Facility staff: facilities that sign in can update their own information.
What the information is not
Open-bed numbers are estimates or facility-reported counts, not live availability. Services, insurance, and admission rules can change. Always call the facility to confirm before you send a patient. The site is a planning tool. It does not decide who is eligible for a placement; the care team does.
